Technically, it means that there seems to be a loop between two caches -
A requests from B, which requests from A.

I've also seen this wrongly reported in the circumstances where two machines
use the same visible_hostname: it's putting the visible_hostname in the
Via: header, rather than the real hostname because it simply calls
getMyHostname().

(that might, or might not be what you've got).
