Try searching the squid-users mailing list archive for messages from me.
I know I answered this at least once before (you just add REG_ICASE to two
lines in acl.c, I think). That message tells the exact lines and includes
more discussion.
Let me know if you can't find it - I have a copy somewhere in this mess...
