If it isn't in the FAQ for Squid 1.1.X then it should be put there ;-)

A icon server is a HTTP server with a icon directory.

The path to this directory is configured in squid.conf:
ftpget_options -n 60 -R -W -p http://www.gc-system.de/icons/

Squid 1.2 uses a internal icon server, solving the icon troubles once
for all ;-)
