You should probably use a lower value for "store_avg_object_size" in squid.conf.
"store_avg_object_size" is explained in squid.conf.default.
Maybe you should increase the # of first level directories as well, I'm not
sure here...

You should really upgrade to a *supported* version: 1.NOVM.21 is >1.5 years old
