I can't address the question of why your configuration doesn't work, or
even whether it should, but I wanted to address one misconception here:

Running squid as httpd_accel will *not* save any bandwidth on your
upstream link. Since squid would be fetching from apache, on the same
box, all URLs requested by outside users and feeding them up your link,
there's no opportunity for savings. All it does is complicate the
configuration.

The main advantage of httpd_accel is when you have a slow and
overloaded web server which can not be changed or upgraded, and you
want to speed up its delivery of static web pages. This doesn't match
the case in your configuration, so you may as well skip it.

I recommend you use squid only as a proxy and not as an accelerator,
and run Apache on port 80 as usual. This will work and be much simpler.
