Ok, recompiled and squid now runs ok, but I get no usersnames. I have
'auth_param ntlm program /usr/bin/fakeauth_auth mydomain\server' as the auth
to use (the only auth - I am not using basic).
How do I actually use fakeauth e.t.c.?? again, I just want usernames.

You can NOT run authentication and transparent mode simultaneously.
you also need to add
--enable-auth=ntlm
to the configure line
