Options to improve hit-rate:
1 Increase cache size
2 Play with refresh rules (try to find out what type of file has the best
   chance of caching and than make sure it is cached as long as possible
   or makes sense)
3 If your users access cache hostile sites that expire files to quickly
   (in your view) override the expire rules (violates standards)
4 If your users access sites that use scripts to get their pages from a
   database with scripts chances are that they are not cached. By playing
   with the caching rules you can get squid to cache them.
5 Get rid of dynamic banners and hidden access logging things by using a
   redirector.

Following this scheme I get a hit-rate of about 50% with a 12GB cache,
about 200 users and a traffic volume of 1 to 1.5 GB a day.
