This indicates the request was denied by your http_access rules.
Verify your http_access rules, and especially so if you are using any
regex based acls as it is very easy to make small mistakes in regex
causing these to match much more (or less) than intended.
Also check your cache.log. There is some special conditions besides
http_access that may access in ACCESS_DENIED and if this is the case
there should be a entry in cache.log explaining why.
