Your problem could be unrelated to the fgets() messages. To confirm/eliminate
ntlm helpers as the problem, check that after running squid -k reconfigure
you get a new bunch of helpers with different PIDs. Also check cache.log for
other interesting messages (eg queue overload) and use cachemgr to see if
ntlm helpers are alive and working after the reconfigure.

Or failing that, elaborate more on "everything stops working". For example, do
clients get timeouts or errors or both? What does access.log show after the
reconfigure?
