Error pages should be root:root mode 644.
What you need to make sure is that the user (or group) squid runs as in
cache_effective_user have execute permissions on all directories up to
the error page
chmod a+x /usr/local/
chmod a+x /usr/local/squid
chmod a+x /usr/local/squid/etc
chmod a+x /usr/local/squid/etc/errors
chmod a+r /usr/local/squid/etc/errors/*
