What Squid version ?

Please check the build date of ncsa_auth.exe, it seems the behaviour of a
too old ncsa_auth version.

A typical problem with ncsa_auth on Windows is the Linux/Windows different
text file format, CR vs CR+LF, but it shouldn't be your case.
