You need to split that in two different ACLs..

acl MONRESEAU src 172.17.0.0/255.255.0.0
acl auth proxy_auth REQUIRED

> acl VLANMEDECIN src 192.168.0.0/255.255.0.0
> acl all src 0.0.0.0/0.0.0.0
> ....
> # autorisation
> http_access allow MONRESEAU
> http_access allow VLANMEDECIN

http_access allow MONRESEAU auth
http_access allow VLANMEDECIN
