Some, squid will think that the files exist when they don't. Thus the
MISS messages in log. It should be replacing the files as found during
the MISS procedure.

Worst case if the SWAPFAIL_MISS persists on same URL, you may have to
stop Squid, delete swap.state and restart. That with result in a longer
than normal startup while it re-scans the entire cache to build an index.
