IIRC, your issue was paying whoever maintains your
DB or radius server. It isn't hard to write a cold fusion
page or java server to modify headers based on
a DB lookup if that is all you need. I'm not sure on
best approach if you want to rewrite headers
in squid but I'm still not even sure which step is of
most concern- accessing a DB or header modification.

It may be that you haven't gotten an answer because
other skimmed your post with same level of
detail and re-wording would help :)
