Like Amos said, you need ICAP or eCap. However, both solutions require a
significant amount of work to make it do what you need.

Since you only want to insert one ad at the top of each page, you also
need to have an algorithm to decide when to insert an ad and
when NOT to insert, i.e. only insert at the the web page level and not
and the sub-page levels.
